Payment

Payment is due at the time of scheduling.

We require tests to be prepaid with a credit, debit, or prepaid card at the time of scheduling to reserve an appointment.

Alternatively, a money order may be mailed to the address below. Money orders must be received prior to scheduling your appointment. Please include a return phone number and the name of the applicant so that we can notify you once your payment has been received and schedule the appointment.
Please note: We do not accept personal checks.
